i think its a combination of many many factors!
the most common thing i see is heat styling/flat ironing/curling/etc. without protecting your hair, all that heat is literally frying your hair. its like going in the sun without sunscreen...it looks good for the time being, but after continuous exposure without protection, it will become dry, brittle, and damaged.
another common issue is over-processing with bleach or other chemicals. your hair is made up of proteins, which are held together with bonds. when you add chemicals to your hair, you're altering the bonds and the proteins, and making them weaker. when you color and bleach your hair over and over and over again without using the correct processes, products, or techniques, the bonds will completely just break, and your hair will break off. literally...
the environment also effects your hair, including water, wind, and sun. bad water, alot of wind, and too much sun will damage your hair as well.

There are 3 main types of hair damage: Chemical (colors & perms); Mechanical (blowdryers and curling irons);and environmental (sun, wind.) I believe that improper blow drying methods can be one of the very worst for damage.

Either straightening it continously, or dying/bleaching it continously. Plus, if you take a deep shower like every two hours, it really dries out your hair!

then bleaches.... those can melt your hair
the longer left on the hair the worse the damage

Probably hair straighteners and curlers they really dry out you hair.
Heat, ie from hairdryers and straightners, too much shampoos etc, general environmental pollution ie car fumes, smoke etc - I believe heat does the most damage
Using numerous chemicals such as hair spray, gel, etc.
constant expose to the sun, salt water, hair dye( bleach), heat (hair drying, crimping, straightening, etc.) everyday shampooing which can cause the hairs natural oils to dry out and cause flaking, etc.............the list goes on and on..........

Hot water, That's why I use cool. :)
